Application to participate in the Boys Baseball, Inc. program for the 2009 season.

**READ CAREFULLY BEFORE AGREEING**
No one is eligible to work with any team in any capacity without approval of the Board of Directors. The Screening Committee will not consider any application unless all questions are answered. You may be accepted or rejected due to information on this form, your past actions and attitudes both on and off the diamond, or any other information developed by the Committee. An application must be submitted every year. A non-refundable processing fee of $5.00 is charged for each application.

I understand and agree that various background inquiries may be made by B.B.I, specifically including inquiries to law enforcement and security agencies. I acknowledge the right of BBI to make said background inquiries generally and law enforcement and security agency inquiries specifically, and I expressly waive any and all claims to privacy with respect to records and information pertaining to me which any such agency may possess. I certify that all statemenst made by me are true and without reservation. If anything contained in this application is found to be untrue, I understand I will not be accepted or, if already accepted, I understand I will be subject to immediate suspension.
